Subject: Dilaudid IV shortage

The IV Dilaudid shortage is worsening and we are falling below a one-month supply. Please help preserve our dwindling IV Dilaudid supply for patients who absolutely require this drug. It may be difficult to switch some oncology patients and a few other select groups, but the majority of adult medical and surgical patients can likely use IV morphine (if IV is absolutely necessary and they are not allergic) or PO hydroCODONE/OXYcodone/PO Dilaudid (if they meet our IV-to-PO guidelines and they are not allergic) - PICK P.O.!

Please work with nursing (as approved by P&T and MEC) to make this switch and preserve our dwindling IV Dilaudid supply. I have communicated this info to nursing leadership, so they are aware that pharmacists will be working with their staff to accomplish this switch.
