Subject: Vancomycin - Automatic PKS consult reminder

Please remember that orders for vancomycin EXCEPT Dr. Ali Hassoun is an automatic vancomycin consult.

If prescriber does not write/enter a PKS consult, the pharmacist seeing the order for vancomcyin should enter a consult.

Example :
Neurosurgery writes order for Vancomycin 1gm IV q12h until drain removed.
Verifying pharmacist response: adjust vancomycin dosing as appropriate AND enter a PKS Consult in EMR.

Again, please enter the PKS consult; if you are unaware of how to enter a NSS/PKS consult into iCare, just ask a coworker.
