Subject: Titration guidelines updated for diltiazem shortage

Pharmacists,

The titration guidelines have been updated due to the diltiazem shortage.

What's changed:
  1. Locations of use for esmolol expanded to PCUs and PSUs (fixed dose with telemetry monitoring)

  2. Verapamil has been added - if ordered, will most likely be at a fixed rate, but may be titrated according to these parameters in an ICU. Fixed rate with telemetry monitoring in PCUs and PSUs
