Hello Huntsville Hospital Main Pharmacists,
The P&T Committee has approved a dosing change for IVIG to be used on ADULT INPATIENT orders.
IVIG will now be dosed based on ideal body weight (IBW) or adjusted body weight (AdjBW) in obese patients.
All IVIG doses will be calculated by pharmacy.
This change will be implemented starting April 2nd.
Dosing Information:
Use ideal body weight (IBW) on all adult inpatient orders
Exceptions:
o Obese patients (total body weight ≥ 130% of IBW) à use AdjBW
o If patients total body weight < IBW à use total body weight
Adjusted Body Weight (AdjBW) Equation:
o AdjBW = IBW + 0.5 ( Total body weight – IBW) = kg

Doses will continue to be rounded to the nearest 5 grams
Pediatric patients and outpatient IVIG orders are excluded
