Subject: Multimodal pain management - avoiding excessive acetaminophen intake

With the introduction of multimodal pain management, occasionally prescribers are ordering additional acetaminophen containing products that would result in the patient receiving more than 4gm of acetaminophen in a 24 hour period.

P&T has approved an automatic adjustment/discontinuation of scheduled acetaminophen if an acetaminophen containing product is ordered. The medical staff at P&T felt pharmacists should leave notes telling the prescribers of our actions. A communication form has been created and can be found on FormWeb under Acetaminophen and under forms. Click here to see the form.

Example : Pt receiving scheduled acetaminophen 1gm PO QID. On post operative day 2, the prescriber writes to DC ULTRAM and Begin NORCO 7.5 mg 1-2 PO q4hr PRN pain. Pharmacist should discontinue the scheduled TYLENOL and leave a note informing prescriber that the scheduled TYLENOL was discontinued.
