03/01/18 RC -- Pharmacist Driven Automatic H2 Block Discontinuation Policy

We have had a long-standing formulary policy in place that the pharmacist may automatically discontinue an H2 blocker if it is being given along with a PPI.  Discontinue 48 hrs after the two being given together, as the PPI does not exert its full effect for a few days and the H 2 Blocker may have been given to cover this time period.  If GI physician prescribed both drugs, contact GI MD to see if H2 Blocker can be discontinued.  Before discontinuing, check to see if H2 Blocker is being given for GI purposes, as there are a few other unusual non-GI uses of these drugs that could be appropriate.
