Robbie has scheduled several dates for a MANDATORY pharmacist meeting:

    * Mon 2/19 3-4 pm
    * Tue 2/20 230-330 pm
    * Mon 2/26 3-4 pm
    * Tue 2/27 245-245 pm
    * Fri 3/2 1230-130 pm<

Please look at your schedule and put ONE of these times on your calendar . (You may not be able to put "5th floor conf rm" in the location box since Robbie has already booked it). If you are off ALL of these times, we can arrange for a one-on-one meeting.

The topics of discussion will be the pharmacists' role in mitigation of the injectable narcotic and IV fluid shortage, removal of OFIRMEV from the formulary, and adherence to the formulary as approved by P&T

As a preview, both P&T and MEC have approved AGGRESSIVE IV-to-PO for narcotics as well as discontinuation of IVs when the patient is taking PO. All of these changes must be accomplished after consultation with the patient's nurse to determine if PO is feasible. If a flowing IV is absolutely required, KVO is now approved to be at 20 mL/hr. You are welcome to begin these practices now as we strive to preserve our dwindling supply of IV narcs and fluids. We will also discuss the removal of OFIRMEV from the formulary as an analgesic in adults (beginning March 1) plus other formulary enforcement issues.

For your review, I have attached an informational sheet that was sent to all physicians (and will go to the HH Board 2/27), a flyer that is going to nursing, and the equivalent IV and PO dosing of various narcotics.
