Subject: Oral Vancomycin

When verifying orders for oral vancomycin please select the capsules whenever it is appropriate. Vancomycin oral solution should be reserved for use in pediatrics or in patients who are unable to swallow capsules. Form Web has been updated to reflect this preference. This is not a change in practice as the capsules have been on the formulary since 2013.

Vancomycin capsules should be swallowed whole. Do not crush, chew, or open.
