Subject: diazepam 10 mg/2 mL injection - CRITICAL SHORTAGE 

The national shortage of diazepam continues. All hospitals (Main, HHWC, and Madison) are down to very few tubexes.

Pharmacists : Please avoid verifying orders for diazepam injection unless it is for status epilepticus. If orders are written for agitation or pre-procedural anxiety, please notify the prescriber of the critical shortage, and request an order for an alternative (p.o. diazepam or another injectable drug).

We have roughly a 2-week supply if diazepam injection.
