Effective immediately, when HEMSI LDB boxes are refilled, dispense only ONE diazepam 10 mg/2 mL tubex. See the attached sheet to see how this will be designated (last column) on the sign-out sheet.

If the HEMSI box has TWO diazepam tubexes, remove one tubex and do a "RETURN" function then place the returned tubex with the C2Safe stock that is marked as RESERVED FOR HEMSI. These returned tubexes are to be used only by HEMSI...they are NOT to be used in dispensing to hospitalized patients or Pyxis machines.

This temporary change is necessitated by the ongoing national shortage of diazepam injection. We are hopeful that more product will be on the market in Early 2018. Let me know if you have questions.
