Subject: Please focus on IV-to-PO conversions

Our IV piggyback status can best be described as 'dire.' We have several plans in place to conserve our dwindling supply, including a massive syringing project that will roll out next week.

One thing pharmacists can do now is convert all possible IV doses to PO . Our goal is to convert as many IV doses to PO as possible, erring on the side of aggressive conversion. If you feel that the patient is on (or slightly below) the borderline of approved conversion criteria and you are not comfortable making the switch, contact the prescriber and determine if conversion is a possibility.

It is imperative that we save every possible IV piggyback bag. Check Sentri7, review profiles, and carefully scrutinize all new orders for the possibility of IV-to-PO conversion.
