iCare has been updated to change ordering Vanco 1 g standard pathways to the PSP-prepared product.  There is a new section in Sentri7 added to the “Drug Shortage Management” tab to identify those orders which are currently Add-Vantage and need to be changed to PSP-prepared doses.  Please work to change these to the PSP-prepared product today at MAIN and W&C.

Effective today:
Vancomycin 1 g ADV vials are on backorder. We started making vancomycin 1 g/D5W 250 mL bags on the robot last night. We had to use D5W because of the NS 250 mL backorder, so the new iCare entry will state '250 mL fluid'. 

After this goes into iCare production today, we'll need help getting the orders re-entered. 

Main & W/C profile Pyxis: Change to vanco 1 g/250 mL fluid. All doses will be distributed from PSP or W/C pharmacy.

Main & W/C non-profile Pyxis: Stay ADV until we run out.

Madison profile and non-profile Pyxis: Stay ADV until PharMEDium product arrives (also in D5W).
