Hospice Family Care Considerations for PSP

  1. HFC has its own on site pharmacy to procure/prepare meds separate from HH pharmacy. HH pharmacy is not able to sell meds to HFC.

  3. The following concentrations of IV’s will be ordered from PharMedium and kept on hand at HFC
  4.  
  5. If an IV preparation is ordered that is not stocked:
    • Home supply may be used if available.
    • Order is sent to ContinuumRx. Phone # (800) 665-2850
    • Search for therapeutically equivalent medication available in Pyxis and see if RN can get new order. Consider IV pushes until ContinuumRX can send the drip.
    • In the EXTREMELY RARE event it is after hours and above options not possible: In an urgent/emergent situation PSP can prepare IVs to make sure the patient is taken care of. Follow #4 and #5 below.

  6. Central/Q pharmacist will verify-send the order to PSP. PSP will prepare using MILT label. Use HH standard concentrations and after prepared, call courier for delivery.

  8. If a CII is compounded for HFC, the drug must be transferred on a DEA 222 form. Let Pamperin or Berkley know if this occurs.
