Subject: Pharmacist Clarification of Home Meds - MANDATORY education sessions
Hi all,
Michele Durda and I will be presenting several mandatory pharmacist education sessions over the next couple of weeks at HH Main and Women & Children's to discuss the recently-approved P&T document (titled Pharmacist Clarification of Home Meds) and a corresponding MRL Addendum form (which will be used and faxed to HCS by our TOC pharmacy technicians). Because TOC is not at Madison at this time, these education sessions will not be presented at Madison at this time.
While the P&T document is relatively self-explanatory, the purpose of the education sessions is to introduce and explain the corresponding MRL Addendum form as it relates to the P&T document, and how pharmacists should address this form when it is received in their work queue. Please know that this is intended to make it EASIER for you to address home med clarifications discovered by our TOC technicians (NOT to increase your workload or clog up your orders queue), while also streamlining our technicians' workflow. In addition, we will be creating a TOC work queue that will allow our TOC pharmacists to address these MRL Addendum forms during our TOC hours. The anticipated go-live date for both the MRL Addendum form and the TOC work queue is Tuesday, 9/19.
