Attachment: Sample Log Database

We will no longer be using the Blood Factors and NovoSeven Log database. With the new process for dispensing medications to non-iCare areas, all blood factors for non-iCare areas will be entered through the "iCare process" where a charge will be generated after "dispensed" is charted. In emergencies or if the blood factor cannot be entered into iCare, please make sure to follow the old manual paper verification process with MILT labels and make sure the paper order gets placed in the clinical technicians bin so it can be charged manually. For areas using iCare, a charge is generated when the nurse charts against the order.

Please continue using the Abilify-Humira Sample Log database and the Invega Sustenna database.

  • When dispensing Abilify Maintena or Humira samples, we are legally required to keep an accurate log of inventory and doses dispensed. See attached for additional information about the Sample Log database.

  • The Invega Sustenna database should be utilized to ensure reimbursement through the voucher program.

Thank you!
Katie Sims
