1/26/26 CMS Age-Friendly Measure - Transfer Med Rec Consult

Pharmacists,

 

Tomorrow1/27/25, we will go live with a new Consult to Pharmacy as a part of the requirements set forth in the CMS Age-Friendly Hospital measure. This consult will be triggered when a patient's location in PowerChart is moved from 1) an ICU to a floor unit OR 2) a floor unit to an ICU. This WILL NOT trigger when a patient's location is changed to a procedural area to avoid unnecessary consults for patients going for procedures and returning to their previous unit. There will be rare occasions where a patient goes to a procedure and then changes levels of care (ex. Floor to CATH to ICU) that the consult will not capture. We have discussed this with PharmIT and there is not a good way to capture this type of movement, as of now. 

 

The consult will have a PPM notification (dummy drug) similar to other consults that we see. Attached are the slides that I presented at HH Main Huddle but to summarize, the expectations for pharmacists are as follows:
  • 1st, 2nd, and 3rd shift pharmacists are expected to complete the consults but these are much less urgent than other consults like vancomycin dosing and can wait until you have time to get to it. We want to complete them within a reasonable time so that we are meeting the standard, but not to the detriment of other patient care priorities. Use your judgement.
  • Review the patient profile for issues related to transfer of location such as ICU orders that should be d/c'd, home medications that should be restarted/discontinued based on the situation, BEERS list medications that may need adjustment, etc. See the slides for more ideas. 
  • Double click the task within the Consults and Interventions tab which will launch an intervention PowerForm. There is a new intervention type called "Age Friendly Med Review". Please check this box and put any details you wish to include in the comments. If you make any interventions, you may also select other intervention types. 
    • Document the "Age Friendly Med Review" whether any interventions are made or not. The standard says we must review patients but does not say we must make adjustments on every patient. These are patients being treated for acute illnesses and we understand that many "potentially inappropriate medications (PIMs)" are necessary for proper treatment of the patient. 
  • Save the intervention and complete the consult
I understand that this is another addition to your responsibilities and that we have many age-friendly Sentri7 rules that are meant to target actionable interventions. Therefore, it is expected that the number of interventions we make with this consult could be somewhat limited. Unfortunately, this is something that we have to do and be able to report on to receive our full CMS reimbursement for 2027 so it is important that we have a high rate of documentation of a completed medication review.
