12/17/25 Urgent: Broselow Tape Recall

See the urgent Broselow Tape recall message below.
We are currently in the process of removing ALL Broselow Tapes from ALL Pediatric code trays at HH Main, HHWC, and Madison Hospital. Once our order arrives in the next few days, we will be adding a PediaTape Quick Tape to each Pediatric code tray as a replacement item.
Similar to a Broselow Tape, a PediaTape Quick Tape can be used to measure length and estimate weight, but does not contain any drug dosing information.
We do have a policy in place that states we should not be using a Broselow Tape for drug dosing information, but due to the serious nature of the drug dosing errors and to be compliant with the recall notice, all Broselow Tapes are being removed from circulation.

Situation:
1st Notification received on
12/15/25 from AirLife that they are voluntarily recalling their Airlife Broselow Rainbow Tape (2025 Edition, 36-23446 Rev 3). This is the 2nd version distributed after the last recall.
Errors are with 1) Vecuronium dosing 2) Flumazenil dosing and color coding 3) Ketamine dosing and printing error
Background:
2nd version of Broselow Tape that has been distributed and pulled due to errors.
Assessment:
Multidisciplinary call and decision made to cease use and discard product per recall. Pediatric Code Cart Weight-based Dosing policy in place for all Madison County Operations (Nursing, Pharmacy) that states Broselow tapes are not to be used for dosing medications.
Recommendation:
Women & Children's nursing and pharmacy departments will use the Pediatape Quick Emergency Tape once it has arrived. In the interim, the following departments will reference back and use the 2019 Broselow Tape (Pediatric ED 5, Pediatric ED 6, PICU Response bag, and WC Pharmacy Code bag). The 2019 Broselow tapes will be removed once the Pediatape is in place.

Future collaboration will take place to determine best steps and devices to use in lieu of Broselow tapes.
