7/1/25 *New* Vancomycin Dosing Guidelines

Good afternoon, pharmacists,
The long-awaited updated Vancomycin Dosing Guidelines have arrived. I have attached the document to this email and it has been updated in several places on Formweb.
There have been several changes made to these guidelines, so please review at your earliest convenience. 
The biggest changes you will notice are that recommendations for initial level collection have changed, the majority of trough-based recommendations have been removed, and the DoseMeRx information has been incorporated. 
Some outdated information has been removed as well.

At this time, we are recommending that the old vancomycin excel calculator no longer be used routinely to ensure continuity of vancomycin dosing. It is still available in some spots on Formweb for reference and for facilities that are not using DoseMeRx.
For now, we will leave the DoseMeRx guide on Formweb in case we need to make other changes to the vancomycin dosing guideline, but the DoseMeRx guide will be removed at a later date.
