5/26/25 Pharmacy PyxisES downtime reference

In an effort to ensure all pharmacy staff has the information needed to escalate a serious Pyxis downtime, the attached reference was developed to give step-by-step instructions. 

 

The reference is intended to assist when an unexpected Pyxis down impacts all or many devices, particularly when critical override is not functioning and users are unable to retrieve needed medications. 

 

During business hours, PharmIT, the Pyxis team, or various leadership will be on campus and can quickly assist, but during alternate shifts and weekends, anyone in pharmacy can initiate the process. Please open a case as soon as it's clear a serious situation is occurring that could result in patient harm.

 

In addition to following all the instructions on the attached guide, use the Pharmacy Escalation Pathway posted in your area to contact someone in leadership. We can help to raise alarm with BD and/or IT, pull in staff to help if needed, and work with nursing units on downtime processes.

Big thanks to the staff that was involved when this happened a while back. They did a great job during a very stressful situation.
