5/7/25 New Sentri7 Rule

Pharmacists,

A new Sentri7 rule has been created in the "Clinical Alerts" tab to help identify patients on ceftriaxone 1 g who are eligible to be increased to 2 g per our recently approved policy. 

The following guidance is listed in the comments section of the rule. 

Per P&T, pharmacists can automatically dose adjust ceftriaxone based on severity of infection--ADULTS only, round up ONLY
Cystitis and cellulitis: Ceftriaxone 1-2 g acceptable, if BMI >30 then 2 g; if ceftriaxone 2 g is ordered, make no changes
Infections outside of the urinary tract or more involved than skin alone: Ceftriaxone 2 g; if ceftriaxone 1 g is ordered, increase the dose to 2 g regardless of BMI
