3/17/25 Cefepime Sentri7 Update

Pharmacists,

 

The cefepime renal Sentri7 rule has been updated to add criteria to show patients on [cefepime scheduled q12 with CrCl >30 OR cefepime scheduled q24 with CrCl >10]. This is designed to catch patients that were initially renally dosed by the provider that may no longer need renal dose reductions. If the pharmacist verifying the order does not adjust the order at the time of verification, we do not routinely tag the order with ',renal' which was already a part of the rule. This should capture those patients should their CrCl improve after verification. Please let me know if you have any questions.
