2/20/25 Vancomycin 1250 mg and 1500 mg doses will now be supplied as Vial 2 Bag at HH/HWC/OPM

Starting today, new orders for Vancomycin 1250 mg and 1500 mg at HH Main, HWC and OPM will be supplied as Vial 2 Bag instead of the premix products. If you need to order a dose of 1250mg or 1500mg, choose the "vancomycin 1250 mg (1.25g) IVPB V2B" or "vancomycin 1500 mg (1.5 g) IVPB V2B", respectively.

Existing active orders for the "vancomycin 1250 mg/NS 250 mL *STD* (PXS)" and "vancomycin 1250 mg/NS 250 mL *STD* (PXS)" will remain linked to the premix for now and will still appear in MedManager. But new orders will need to be entered as Vial 2 Bag. Once, there are no longer any active orders for the premix, PharmIT will unflex it so that there is only the V2B option at HH Main/HWC/OPM for 1250 mg or 1500 mg doses.

This change is in response to a shortage of the premix product.
