1/30/25 Vancomycin calculator

We have uncovered an issue with our HH vanco calculator.  Hopefully, most of you have moved on to DoseMeRx and the 'bug' in our homegrown program will have no impact on your vanco management.    Riley is working on the issue and it is highly likely the calculator will be taken down while repairs are made.    Just wanted to make everyone aware in case they encounter a dead link to the calculator.  If you have the calculator saved to your files, please do not use the calculator until we can send out the fixed version.  We will send updates as they are available.

 

As a reminder, DoseMeRx should be used for the overwhelming majority of our adult patients.  Exceptions are Intermittent HD and Peritoneal Dialysis where we have always used protocols outside of any calculator.  The old calculator is still used for CRRT patients to determine the AUC based on levels (Step 3).  If you have levels resulting on a CRRT patient and the old calculator is not available, the calculator section of Micromedex has an AUC calculator that looks very similar to the Step 3 of our calculator and can determine an AUC based on two levels.
