08/08/17 BG -- NEW REQUIREMENT for refilling return bin contents back into Pyxis

Subject: NEW REQUIREMENT for refilling return bin contents back into Pyxis

Effective immediately for narcotics and non-narcotics:

When you use the "refill" function to refill the contents of the return bin back into the Pyxis machine, you MUST scan the barcode of each medication individually. (example: If there are two NORCO 7.5 in the return bin, you must return them by scanning each barcode.....this will create TWO refills for this action.)

Please note that continuous electronic auditing will be done to assure compliance.

This requirement is being implemented to reduce/eliminate refill errors associated with these medications. We have had multiple QRRs in the last few months related to misrefills  (a NORCO 7.5 and a NORCO 10 were removed from the return bin and both refilled into the NORCO 7.5 pocket.)

As a reminder.......
Please make sure to:
1. inspect the medication's packaging to assure that it is intact
2. assure that the medication is not expired or about to expire
3. SCAN each medication's barcode to refill.
NOTE: if the medication will not scan, you must bring it back to the pharmacy....do NOT refill it back into the machine if it will not scan.
