1/16/25 New PO Chemo Alert and FormWeb Updates

Good morning all,
As part of my major project, I have been evaluating the reinitiation of home PO chemotherapy for patients admitted at HH Main/Madison. A lot of this evaluation has centered around the ordering physician group (hospitalist, oncology, etc), the appropriateness of the order, and the verification process.
To assist in the verification process, two changes have been implemented: a new Cerner alert in Med Manager and updates to FormWeb drug listings.
Custom Cerner alert
- A new custom Cerner alert will fire when an oral chemotherapy agent is being verified. It is similar in structure to our dose range alerts, but with a custom message and action buttons.
- We have built and attached the alert to a large list of PO chemotherapy. However, there are new PO chemotherapy agents approved by the FDA constantly and we are not able to keep up with Cerner builds in real time. This should be a tool to assist the pharmacist as a last-line catch before verification and not the only check in place for proper reinitiation of these agents.
FormWeb updates
- FormWeb has been updated to include a statement on PO chemotherapy agents as well as our hormone therapies to remind the pharmacist of the different policies for verification of these agents.
- We are working to continually add more PO chemotherapy agents as they go through P&T. We have updated around 100 drug pages in FormWeb. This is by no means an exhaustive list and other resources should be utilized if the drug is not on FormWeb.
A few reminders with this implementation:
- Even though there are many PO chemotherapy agents built in Cerner, they sometimes fail to cross over as a built drug when re-ordered from a home medication list. Be diligent about checking agents that cross over needing to be product assigned.
- Continue to be proactive looking up unfamiliar drugs in not only FormWeb but other drug resources when verifying orders. There are new agents approved constantly by the FDA and it can be difficult to keep up!
The go-live for this alert is 1/20. FormWeb has already been updated. I will be presenting these updates in Huddle on 1/22 as well. Please let me know if you have any questions or concerns.
