1/15/25 Consult to Pharmacy Notification - Vancomycin Not Given

January 15, 2025

Pharmacists,

Today the "Consult to Pharmacy Notification" for vancomycin charted as "not given" will officially go live at HH Main only. The notification seen below will be triggered once a vancomycin dose is charted "not given". It will appear in the corresponding PPM of the patient's location. It should be determined by the verifying pharmacist if not giving the vancomycin dose appears appropriate. If determined to be appropriate, all you need to do is void the dummy order. There is no task or consult attached. If determined to be NOT appropriate, the nurse should be contacted to clarify why it was charted as "not given" and take any necessary steps to correct the situation. Once the charting has been addressed and corrected if needed, you can void the dummy order.

This is not expected to have much impact on workload. During the pilot period, the notification was being triggered 1-2 times per day on average. About 1/3 of these notifications were potentially actionable. Please let me know if you have any questions.
