1/14/25 Vancomycin 1250 mg doses will now be supplied as Vial 2 Bag at HH

Starting today, new orders for Vancomycin 1250 mg at HH Main will be supplied as Vial 2 Bag instead of the premix product. If you need to order a dose of 1250mg, choose the "vancomycin 1250 mg (1.25g) IVPB V2B" IV Set:

 

Existing active orders for the "vancomycin 1250 mg/NS 250 mL *STD* (PXS)" will remain linked to the premix for now and will still appear in MedManager. But new orders will need to be entered as Vial 2 Bag. Once, there are no longer any active orders for the premix, PharmIT will unflex it so that there is only the V2B option at HH Main for 1250 mg doses.

 

This change is in response to a shortage of the premix product.
