1/8/25 LORazepam injection shortage

Our wholesaler is out of LORazepam injection and the manufacturer is not currently shipping any product. We have continued to place orders and have communicated with both the wholesaler and the manufacturer, who states that an order should be shipped to us either this Friday or this coming Monday. However, with the expected weather issues, we are not confident that we will receive this order in time to prevent exhaustion of our current supply.

 

In light of this situation, the P&T Committee has approved restriction of LORazepam injection in adult patients to treatment of active seizures and for palliative care/end-of-life situations; there will be, however, NO restrictions placed on usage in pediatric patients. We have a good supply of both diazePAM and midazolam injection. Orders for LORazepam injection in patients not meeting this criteria may result in delay of care as pharmacists will reject these orders and contact the prescriber for an alternative.

 

Thank you in advance for compliance with these restrictions. We will notify you as soon as the supply of LORazepam injections returns to normal. DAVID
