11/20/24 Dilaudid 5 mg / mL PCA

Beginning today, a new Dilaudid PCA concentration is available in the **Pharmacy Only ADULTS Powerplan**
This 5 mg/mL concentration was created to facilitate management for a specific patient requiring extremely high dosing that cannot be accommodated using the 1 mg/mL concentration (at home she is on a 10 mg/mL PCA, with a 1.5 mg/hr continuous rate and 10 mg q30 min demand dose)
In the future, we may have other patients that also require this concentration -

 

To improve safety with this high risk concentration, we have done the following:
1. It can only be ordered through the  **Pharmacy Only ADULTS Powerplan**
2. In Cerner and in the pump, it is called Dilaudid *MAX HIGH*; please be sure the nurse knows to select the correct pump entry
3. Please use only in cases where a lower concentration cannot be used - there is a hard stop on the pump that prevents programming a dose that exceeds 1/3 of the syringe volume (therefore, this patient's dose could not be programmed using the 1 mg/mL concentration - we ended up giving the continuous rate through a separate infusion)

Please let me know if you have any questions. This patient has an open chart alert with instructions on entering the patient's PCA orders; it will be updated to indicate that the 5 mg/mL concentration should be used
