11/13/24 KCL 20 mEq Back Order Plan

November 13, 2024

Due to the ongoing critical backorder of KCL 20 mEq/100 mL premix bags, we've made adjustments to several PowerPlans including the electrolyte protocol. At HHM/HWC/HBH/OPM, Doses that were previously ordered for 20 mEq bags will now be ordered as multiples of 10 mEq bags (x 2 doses OR x 4 doses depending on the PowerPlan.) Additionally we've updated the 20 mEq order sentences accordingly. See details below, and please let "pharmacy it group" <grp_pharmit@hhsys.org> know if you have any questions or concerns.

Good afternoon,

As discussed on Wednesday due to the ongoing critical backorder of KCL 20 mEq/100mL premix, we plan on updating the KCL 20mEq orders to 10mEq (x2 doses) on the following PowerPlans:
CVSURG Electrolyte Replacement Orders CVICU
CVSURG Electrolyte Replacement Orders PCVU 
ED ADULT Diabetic Ketoacidosis (DKA) Orders
ONCP GYN Outpatient Electrolyte Replacement
TP Legacy of Hope (LOH) Organ Donor ORDERS

AND creating new backorder subphases with 10mEq (x2 doses or x4 doses*) for the following PowerPlans (only flexed to HHM/HBH/HWC):
HOSP Electrolyte Replacement ORDERS for Continuous Renal Replacement Therapy (CRRT)
HOSP Electrolyte Replacement ORDERS for Hemodialysis (HD)
HOSP Electrolyte Replacement ORDERS for patients NOT on HD or CRRT
 
Additionally we will updating the order sentences outside of PowerPlans for HHM/HBH/HWC.
*Note: all orders converted will retain the current total dose and administer over time. Ex: KCL 20 mEq IVPB Once Administer over: 2 hours --> 10 mEq IVPB q1h x 2, Administer over: 1 hour (each)

We plan to go live with these changes on the afternoon of Tuesday, November 12th.
