Pharmacists,

 

Due to the increasing frequency of backorders, including the substantial fluid shortage issue we are currently dealing with, PharmIT has created a new comma phrase to use for backorders.

“**Product interchanged due to Critical Backorder per P&T**”

 

Another comma phrase was added due to some recent med errors where nursing inappropriately administered local anesthetics intended to be given by a provider at bedside for a procedure. This comma phrase will allow you to quickly add a comment to the order to help indicate to nursing that this is not intended to be given until the provider is ready for the procedure. Please use this when verifying local anesthetics like lidocaine with epi, intended for procedural administration.
