If you enter an order with the “RX” frequency and the PRN field checked (ex. Banana Bag, PCA), iCare will not assign a time to the order for charting purposes.  Therefore, in order to chart the given medication, you will need to enter a TIME of medication dispensing and then select the DISPENSED charting action prior to saving.  Screenshots below:

NOTE: Continuous IV Medications such as Banana Bags must have a “PRN” flag active in order for these to show up properly on the IV Worklist for charting purposes.

There seems to be some confusion related to printing of the "verified/send" order scan during the manual order process.

To clarify:

  1. If the medication order is entered into iCare, do NOT print the order scan, regardless of location

  2. If the medication is demand picked from the carousel, the order scan is printed only if the patient is somewhere other than the ED.  Orders do not need to be printed for ED patients because the ED charge clerks will charge for the product.

Don't forget that today is the go-live for Main for the new manual order process.  Please note that the Pharmacist charting instructions (attached) have been updated.  Continuous infusions do not appear on the regular Med Wrklst.  Therefore, an additional button for the IV Wrklst has been added to the Medication Profile screen, so that you can chart against these items.

Pharmacists,

Based on some concerns about the August 1 start date coinciding with the Pharmacist coverage changes, we are making some adjustments to the roll-out of the new manual order process.

 

Update

  1.  The iCare changes will still go live on August 1
  2.  HHWC and Madison will go live on August 1
  3.  Official go live for Main will be delayed until August 15; Main Pharmacists may choose to use the new process starting August 1, if desired

 

Key Point:  If an order is entered for a patient in a non-iCare area, the dose MUST be charted by the pharmacist to remove the charting action from the worklist.  This is a SAFETY issue.  Even if the order is discontinued (unless the d/c is back-timed to the due time), the charting action will remain on the worklist.  This could potentially result in the medication being administered again after the patient is admitted.  I am aware of at least one error where this occurred. 

 

By documenting "dispensed", you are not indicating that you are the pharmacist that physically checked the medication product.  You are only indicating that the medication is being sent from pharmacy.  It clears the worklist and generates the medication charge.  This is the same as the "billed" indicator for bulk medications and the FLSTK order for medications removed from nonprofile PYXIS machines.  It shows a charge was generated, but it does not indicate the medication was given.  Administration documentation would occur in Wellsoft, SIS, or other system used by the department.

 

Please also note that the Blood Factor Log will need to be maintained until this process is fully implemented.  If a Blood Factor is not entered as an order and charted against, it must be logged in the database or the charge will not be captured.

Medications requiring preparation will be entered into iCare by the pharmacist and charted against.

*Notifies dispensing area
*Generates medication charge
