8/26/24 Transfer Medication Reconciliation Consult

Good morning,

 

A new “Consult to Pharmacy” has been created to identify patients who have transferred from an ICU to the floor. The consult will create a one-time task similar to a dosing weight review.  The consult will not generate a dummy drug. A screenshot of this task can be seen below with the designation, “Patient transferred from ICU.” The consult and subsequent task will only fire when a patient leaves an ICU at Main and goes to a step-down at Main.  If they go to a step-down, procedural area, or any unit at Madison or W&C, it will not fire.  So, this should be a Main only consult. This is part of my MidYear project, which is looking at transfer reconciliation.  At this time, I will be reviewing and completing these tasks.  THERE IS NOTHING FOR THE UB RPH TO DO.  I will do my best to clear off these tasks daily (except on weekends when I am off) so they do not clutter up the task list.
