8/6/24 Voriconazole default weight change for ADULTS Effective today
August 6, 2024

The Default weight in the Dosing Calculator has been updated: Similar to the updates to Acyclovir/DAPtomycin, Cerner will now default to the correct weight (Actual vs Adjusted depending on if TBW/IBW is >130% or BMI >30) during order entry.

 

Note:  The default weight calculation formula has only been applied to the ‘VFEND’ and ‘voriconazole’ synonyms.  The ‘VFEND (PEDS)’ and ‘voriconazole (PEDS)’ synonyms will not utilize this default formula.

 

Please continue to review weights being selected by Cerner during verification and let us know if there are any issues.
