7/24/24 Standardization of links to the Antimicrobial Dosing Guideline and Renal Adjustment Policy for Adults

Hey all,

 

Thanks to AMT, Brian, Cappy, Travis, and others, we have made updates to ensure access to the Antimicrobial Dosing Guideline and Renal Adjustment Policy for Adults is readily accessible and consistent.

 

On Formweb, the link has been standardized to “Antimicrobial Dosing Guideline and Renal Adjustment Policy (Adult)” on the Unit-Based Pharmacist ResourcesAntimicrobial Stewardship, and Huntsville Hospital for Women & Children subpages

  • Just CTRL+F “renal” on any of these Formweb pages and the link should come up quickly

 

If you search any antimicrobial on Formweb that requires renal adjustment, you should now see a link to the same policy and standardized CrCl calculation used in Cerner, Sentri7, and the Vanc Excel.

Example

If any drug pages happened to have a renal dosing table, that information has now been updated to match the policy.  In addition, we confirmed all tables on Sentri7 were already correct and up-to-date.
