7/24/24 Standardization of links to the Antimicrobial Dosing Guideline and Renal Adjustment Policy for Adults
July 24, 2024
Hey all,
Thanks to AMT, Brian, Cappy, Travis, and others, we have made updates to ensure access to the Antimicrobial Dosing Guideline and Renal Adjustment Policy for Adults is readily accessible and consistent.
On Formweb, the link has been standardized to “Antimicrobial Dosing Guideline and Renal Adjustment Policy (Adult)” on the Unit-Based Pharmacist Resources, Antimicrobial Stewardship, and Huntsville Hospital for Women & Children subpages
If you search any antimicrobial on Formweb that requires renal adjustment, you should now see a link to the same policy and standardized CrCl calculation used in Cerner, Sentri7, and the Vanc Excel.
If any drug pages happened to have a renal dosing table, that information has now been updated to match the policy. In addition, we confirmed all tables on Sentri7 were already correct and up-to-date.
Please let us know if you have any questions or concerns about these changes.
Thanks!
