Subject: New CATHFLO order set and process - goes live TODAY
Attachment: Cathflo

The new alteplase (CATHFLO) order set for catheter declotting goes live TODAY (see attached nursing flyer)

It is designed to streamline the process for nurses to request CATHFLO for catheter declotting.

Changes you will notice (after iCare changes go live some time today):

  1. PRN MEDICATION orders for CATHFLO will no longer be created when the PICC line care order set is entered
  2. Order sets which authorize the nurse to use CATHFLO for declotting as needed (ex. PICC line, Routine Oncology Orders, others) will generate a NURSING order instead of a MEDICATION order (these orders will not require verification)
  3. When the nurse actually needs CATHFLO for declotting, he/she will complete the new order set, indicating the number of lumens requiring declotting
  4. The CATHFLO order set will create a separate NOW order for each lumen requiring declotting (PORT 1, PORT 2, PORT 3)
  5. Verify each order (up to 3 at a time)
  6. A label will print out for each order (up to 3 at a time); Send a vial of CATHFLO for each label
  7. The nurse should chart against each one of the orders (up to 3 at a time)

Please note:  These labels will only print when the CATHFLO is NEEDED; Send the CATHFLO when the label prints; Do NOT wait for the CATHFLO to be resupplied

Let me know if you have any questions about these changes or have recommendations about how to improve the process.

Thanks,
Michele
