Medications not yet evaluated by P&T are considered NON-FORMULARY . . . . . Always check 2 unique patient identifiers - NAME and DATE OF BIRTH - at every step! . . . . . Please be sure to document all clinical activities daily.

Hey all!

Since it’s our process to create interventions for patient’s own medications (POM), a new option to match has been added to the Pharmacy Interventions PowerForm.  Thank you very much to Morgan for this suggestion!
