Subject: natalizumab  (Tysabri)  verification by Q pharmacists

HH Main Pharmacists:

The "Q" pharmacists and central pharmacy staff (on the weekends) will now be responsible for verification of Tysabri orders for Outpatient Medical (OPM). It is no longer necessary for PSP to retain documentation containing the medication LOT # upon dispensing to comply with the TOUCH Prescribing Program. Please confirm the following items are completed and faxed to HCS:

 
1) Pre-Infusion Checklist (make sure patient has answered "NO" to all pre-infusion questions; if any are "YES" OPM must contact MD before verification)
2) Patient Authorization for current date (must be renewed by MD every 6 months)
3) Order/Prescription
4) Home med sheet (to check for immune suppressing medications; if found please contact OPM & MD for approval before verification)
