Good morning,

Dr. Malavade would like to begin dosing his own vancomycin like Dr. Hassoun. (They share similar dosing philosophies)

Beginning today, please sign off of all Dr. Malavade's vancomycin orders and defer further dosing and monitoring to him.

Moving forward, please use the same process for vancomycin orders written by Dr. Malavade as you would if Dr. Hassoun had written the order.
