Subject: DAPtomycin (Cubicin) Updates in Cerner Effective Today

 

A few adjustments have been made to DAPtomycin (Cubicin) orders to improve quality of life for pharmacists by helping reduce the number of times pharmacists may need to manipulate orders for this medication:

 

  1. Default weight in the Dosing Calculator: Similar to the recent update to Acyclovir, Cerner will now default to the correct weight (Actual vs Adjusted depending on if TBW/IBW is >130% or BMI >30) during order entry.

 

Note:  The default weight calculation formula has only been applied to the ‘Cubicin’ and ‘Cubicin DoT’ synonyms.  The ‘Cubicin PEDS’ synonym will not utilize this default formula.

 

  1. The NOW priority has been removed from DAPtomycin orders in the HOSP Adult DAPtomycin (CUBICIN) Orders PowerPlan

 

  1. FormWeb <https://formweb.com/master/display.php?dir=hhsys&thedrug=DAPtomycin>  has been updated to reflect the adjusted body weight cutoffs
