06/27/17 RC -- thyroid desiccated Dose Rounding

A dose rounding protocol for approximately 20 odd doses of desiccated thyroid tablets (i.e. 32.4, 48.75, 65, 97.5 mg, others) was approved, the protocol also includes unusual higher doses where 2 smaller strength tablets would be sent for the larger strength (i.e. 2 x 120 mg for 240 mg tab, 120 mg plus 30 mg for 162.5 mg tab). Formulary strengths stocked are: 15, 30, 60, and 120 mg tabs.
