Good morning,

At the most recent P&T meeting, an update to the ADULT Zosyn Extended Infusion PowerPlan to change the bolus and maintenance doses from 3.375 g to 4.5 g was approved. (Zosyn maintenance doses will continue to be infused over 4 hours, the only change is the dose being administered.) 

I have attached an educational handout and an SBAR assessment to explain why this change occurred and how this will impact Zosyn dosing moving forward. The go-live date for this change is October 18th.
