Pharmacists,

When signing off of a PKS consult to defer further dosing and monitoring to a physician (ex. vancomycin dosing and monitoring for Dr. Hassoun's patients) please complete the plan of care by indicating: DC'D/REFER; RESOLVED; _________ (Medication) in the appropriate boxes. Also, please write a sign off note in the chart to include the following information:

 
1) Dose of vancomycin the patient is currently receiving

2) Further dosing and monitoring will be deferred to Dr. __________

3) "PKS to sign off"
