Happy Friday Afternoon

The methadone reference sheet has been updated with the following changes:
*Reminder to verify use/dose for pain management with PDMP (look out for LA/SA errors); we had a patient who was documented as taking methadone at home, when she was actually taking metoprolol
*Recommendation to consider dose reduction after 2 days of missed treatment in the setting of vomiting or questionable compliance (such as a holiday take home dose)
*Clarification on use for opioid use disorder (OUD) - there are no restrictions on the use of methadone for OUD in the hospital; however, if methadone is being initiated in the hospital, it is critical that the provider arrange for outpatient therapy with a methadone clinic PRIOR to discharge to prevent treatment interruption. If asked, recommend a case management consult for this
*Addition of some dosing recommendations for initiation of OUD treatment
