Subject: Fentanyl Patch Home Med Conversion Fix
We have previously received reports re: issues related to converting home fentanyl patch medications to inpatient orders with the Admission Reconciliation process. Specifically, the dose/amount and product would not convert correctly when selected to be continued from a home medication order. The primary reason for this was due to us hiding the synonyms from use on the inpatient side in order to encourage the use of the HOSP fentanyl (DURAGESIC) Patch Powerplan.
In order to fix this home medication conversion issue, we will be un-hiding the synonyms later today and adding a comment to stress to providers to use the Transdermal Patch Orders Powerplan when ordering for an inpatient. This order includes a comment to warn the verifying pharmacist if this sentence is used (screenshots below). We are hopeful a provider will not select these sentences given the instructions presented, but we unfortunately cannot stop them from selecting entirely.
By un-hiding these synonyms, the admission medication reconciliation pathway will be corrected.
