Subject: Updates to HOSP Common Concerns Power Plan
Good morning,
I am emailing you all about changes that are being made to the "HOSP Common Concerns" power plan. As part of my major project, we are making changes to the PRN anti-hypertensives in this power plan. Current literature suggests that the over-treatment of acutely elevated blood pressure in the inpatient setting leads to a longer duration of stay and an increase in adverse events. Based on recommendations from the updated literature, we have obtained approval to increase our current treatment parameters for general medicine patients.
Currently, the power plan instructs nurses to treat as-needed for a systolic blood pressure of greater than 160 mmHg. This parameter will change to treat at a systolic of greater than 180 mmHg. Additionally, we are making a few other changes to the comments and frequencies, and encouraging physicians to utilize the oral clonidine as a first line option. All of the approved changes are in the document attached above. These changes apply to the "HOSP Common Concerns" power plan only, and do not include any ICU or anesthesia order sets.
The changes will go live on Monday, June 6th. Nurses and physicians have been educated on these changes, but we do anticipate questions since it is a nurse-driven protocol and requires physicians to "re-favorite" aspects of this power plan.
