Pharmacists,

Dr. Siddiqui has requested the we follow her patients when vancomycin, aminoglycosides, and/or polymyxins are prescribed as we do for all other ID physicians other than Dr. Hassoun. She is going to decide after a trial period if pharmacy will continue to provide this service. This will be an auto-consult, so she does not need to write PKS to dose. It is also appropriate to automatically adjust Dr. Siddiqui's antibiotics, as well as the other ID physicians except Dr. Hassoun, for renal function using the P&T approved guidelines. The new process for Dr. Siddiqui will start on June 9th. She will continue to manage patients that were initiated on any of the above agents prior to June 9th. Please make a folder for the above mentioned antibiotics and place the patient on the list as usual starting on June 9th.
