Clarification: 
 
The new frequencies are not 'on call', they will still task the nurse at 1800.
 
If a patient finishes dialysis early and the nurse sees the frequency is 'upon return to unit' and wants to give the vanco before 1800, they can do that and override the warning about it being given early.  If the nurse wants pharmacy to reschedule the vanco for the time it is actually being given, DO NOT reschedule. Rescheduling opens up the possibility of order entry error.  
 
**This underscores the importance of triaging your vanco/aminoglycoside patients first thing in the morning. If an early a.m. level necessitates a dosing change, make that dosing change a priority in case the patient does have an early HD session and the RN gives the dose early. i.e. don't put off entering the dose thinking you have until 1800.**

Due to confusion about where post-dialysis antibiotics should be administered, we have created the following new frequencies to be used for the majority of patients:
 
After Dialysis MWF-Upon Return to Unit
After Dialysis TTSA- Upon Return to Unit
After Dialysis Once- Upon Return to Unit
 
Please note that there are some exceptions where these frequencies should NOT be used, for example for "unattached" dialysis patients that are admitted directly to dialysis from ED then discharged without going to the floor/ICU. For these patients if a medication should be given in dialysis, the ONCE frequency can be used.
