Subject: After Dialysis Frequencies

Due to confusion about where post-dialysis antibiotics should be administered, we have created the following new frequencies to be used for the majority of patients:
 
After Dialysis MWF-Upon Return to Unit
After Dialysis TTSA- Upon Return to Unit
After Dialysis Once- Upon Return to Unit
 
Please note that there are some exceptions where these frequencies should NOT be used, for example for "unattached" dialysis patients that are admitted directly to dialysis from ED then discharged without going to the floor/ICU. For these patients if a medication should be given in dialysis, the ONCE frequency can be used.
