05/23/17 RC -- methenamine combination update

Due to UTA, the formulary workhorse for this drug group being on long term backorder, we will transition to Urogesic Blue as the new workhorse agent. The several similar products will be interchanged to Urogesic Blue. 

Urogesic Blue and UTA are similar in content.
